  • 90 Minute Weekly Parent Child Home Visits

  • Socializations or Field Trips Twice Monthly

  • Health & Nutrition Screenings

  • Links Children to Head Start

  • Developmental Delay & Disability

ENROLLMENT REQUIREMENTS

  • Proof of income (W-2, 1040, DHS or SSI Statement)

  • Birth Certificate/Birth Record

  • Immunization Record

  • Social Security Number

  • Child Insurance Card

  • Current Physical-Well Baby Check
